Support, education and assistance for family members and individuals who provide care for adults over the age of 60 or for an older adult with age-related diagnoses or disabilities. Services are available in Carver, Hennepin, Ramsey and Scott Counties.
Services:
- Comprehensive annual assessment to determine needs
- Individualized plan of care
- Information, education and referral
- Caregiving coaching, consultation and ongoing support
- Individual counseling
- Facilitation of family meetings
- Respite
Responsibilities/Fees:
Sliding fee scale for contributions. No one will be denied service for an inability to pay.

Monday – Friday, 8 a.m. – 5 p.m.
