Senior Care Management
Catholic Charities’ Senior Care Management helps the elderly live as independently and as safely as possible in their communities. The program provides in-home intensive care management to elderly people in Ramsey and Hennepin counties. The program’s goals are to enhance the quality of life of elderly adults, preserve their dignity and support their autonomy.
Independent living can be challenging for low-income older adults who lack support from family members or the community. Isolated older adults often require additional assistance to access social and medical services, find transportation and maintain their homes and finances.
Professional care managers assess clients’ needs, develop individualized care plans, coordinate services and advocate on clients’ behalf. Volunteers provide transportation to appointments and help older adults with tasks such as grocery shopping.
Services: - Comprehensive in-home assessment to create an individualized care plan
- Referral and coordination of services in all areas of participant’s life
- Advocacy on behalf of the program participant
- Close collaboration with other service providers to coordinate and monitor care
- Reevaluation of service needs on a regular basis to meet changes in participant’s life
- Online forum and network for caregivers
2008 Program Outcomes:
- 500 individuals were served through the Senior Care Management program
- Of clients served, 95 percent were not placed in long-term care
Responsibilities/Fees: There are no fees for Senior Care Management services. Participants are low-income elders and older persons with disabilities who receive funding through one of several home and community-based programs.
